Nurse Manager - Facility Connected Care Manager

The Nurse Manager - Facility Connected Care Manager will function in a program manager capacity and will be responsible for all connected care programs at the Pittsburgh VA Medical Center and associated CBOCs. This position is critical for managing the expansion and evolution of virtual care at the Medical Center, which is becoming increasingly important in the current and post-COVID environments which will require increased utilization of virtual modalities for routine care delivery. The incumbent will be accountable to the Group Practice Manager and will collaborate closely with the Facility Connected Care Medical Director. The incumbent will be responsible for developing, implementing, maintaining, and providing leadership, strategic direction, and program oversight of the Connected Care Program at the Pittsburgh VA Medical Center. These responsibilities will require the ability to apply the specialized principles and practices of health care management. The incumbent will serve as the primary administrative contact and as such, will be responsible for planning, organizing, developing, directing, and evaluating operations. The incumbent will be expected to demonstrate experience through direction and creative approaches to telehealth, telemedicine, and virtual care management of complex patient care. Care will be provided in the least restrictive, most cost-effective manner through use of the clinical practice guidelines and clinical pathways. Evidence of health promotion and chronic disease indices will be monitored through the clinical practice guidelines and performance measures. The incumbent will hold programmatic responsibility for the VA staff, services, and/or processes within specified area(s) of assignment. The incumbent will collaborate with the Chief of Staff and VISN 4 Telehealth Program Lead to ensure adequate staffing, competency of staff, resolution of personnel issues, and efficient operation of the Connected Care Program. She/he will execute leadership characterized by substantial and continuous responsibility and accountability for integrating virtual care programs cross service, facilities, and/or discipline lines, and influence organizational mission and health care. In addition, she/he will; Educate and evaluate staff in the application of the critical thinking process as applied to the Connected Care Program. Use advanced clinical and telehealth/telemedicine knowledge and other Connected Care expertise to promote staff involvement in planning, decision-making, and evaluating outcomes. Lead the telehealth and virtual care delivery initiatives within the organization and in collaboration with Medical Centers across VISN 4. Lead by example a manner that demonstrates accountability and adherence to the highest standards of professional practice and patient care. Function as facility telehealth and My HealtheVet subject matter experts regarding applicable telehealth knowledge, skills, and communicates and enforces nursing and facility policies and procedures. Incorporate the eight (8) core competencies of the High-Performance Development Model (HPDM) in practice and monitors its use for employees and programs. This is a Supervisory position. The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
