Registered Nurse - Home Based Primary Care (HBPC)

The Registered Nurse - Home Based Primary Care (HBPC) provides a full range of nursing care to patient in the home setting while upholding the highest levels of diligence and utilization of the nursing process. The incumbent promotes continuity of care through the development of an individualized interdisciplinary plan of care in collaboration with the patient, family, and the rest of the HBPC team; involving community resources as appropriate. She/he continuously identifies opportunities to improve patient care through monitoring, analysis, and evaluation of care outcomes. The incumbent must have a valid Pennsylvania driver's license as this position requires extensive travel to and from the patient's home. The preferred incumbent will have 3-5 years of successful, recent nursing practice, which must include experience with Acute Care, Home Health, or similar setting. The selected incumbent's clinical duties will include, but are not limited to; Completing initial assessments and/or reassessments of the patient's condition and response to treatment and other clinical findings; initiating follow-up as needed. Assessing patient learning needs, developing and implementing a collaborative plan, and instructing patients and families on self-care and treatment measures; modifying approaches as indicated. Recommending appropriate action as needed for quality patient care which may include home visits by other Medical Center staff or referral to specialty clinics for evaluation. Adhering to all applicable Health Insurance Portability and Accountability Act (HIPPAA), Protected Health Information (PHI), and Personal Identifying Information (PII) rules and regulations when it comes to protecting the patients' information across all channels; electronic, verbal, written. Demonstrating effective written and verbal communication skills and professional behavior, maintaining effective collaborative relationships with both internal and external stakeholders. Participating in the orientation to home care of other disciplines, i.e. Physicians, Psychiatrists, Psychologists, Residents, Medical students, Chaplain students, etc. Serving as a mentor to Nursing students, when necessary. Demonstrating the knowledge and skills required after completing HBPC competencies to meet specific skilled nursing needs in the home such as; Lab draws Changing of Foley and Supra-Pubic catheters Monitoring vital signs Administering and managing medications and pill box fills Administering injections Anticoagulation management and use of CoaguChek meter Oxygen safety, nebulizer and inhaler education Use of Pulse Oximeter Diabetic and glucometer education Infection control and education Adhering to all Medical Center and service line mandated safety policies, procedures, and protocols. Demonstrating the ability to work effectively with all members of the multidisciplinary care team in order to accomplish the Medical Center's mission, vision, and highest quality of care for the Veteran population. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred
Qualifications:
MSN Degree; Home Health and/or VA Ambulatory Care/PACT experience; Nursing Certification in Gerontology, Ambulatory Care, Case Management, or other related nursing specialties. Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
